This can be made in only 3 steps!
An extrusion, a revolve & an extruded cut.
If you can't make this model, check out the first couple of lessons in the built-in tutorials that come with SolidWorks.
(Check the attached image to see to find them, but depending on your settings, you might find them in the Help menu).
I would revolve the cylindrical features first. The I would use a reference plane offset 30mm from the shoulder to sketch out the "fork" profile, then extrude that. What you have shown is missing some dimensions though, unless they are expecting you to make assumptions.
Are you asking us to model this part for you? Because this looks like homework.
