How to add custom tool bar in solidworks 2011?

Not sure if you can add a custom toolbar.. but you can add commands to and existing toolbar.

If you are using the command manager, then you can add your own custom tab with whatever buttons you want. Go up to Tools>Customize. Right under the Command Manager, you will see activated tabs and unactivated tabs. There is also a "New Tab" option that will create a blank tab. Click that, then add features to your hearts content.
