Thread mate wont line up

I have been trying to use the screw mate for a while but for some reason every time I try it the threads end up misaligned by a random amount as show in the photo, the thread sizes are the same for both and I checked multiple times that I have the right distance per revolution set and I just cant figure out what's wrong. Any insight would be much appreciated thank you.

Are you sure that you have chosen Distance/revolution and not Revolutions/mm?

Also, remember that you need to align the threads perfectly (best done using mates) before applying the Screw-mate.

As an alternative to using the "bolt" mate which itself requires an initial sync (another mate you place "just for positioning") to get the threads to match, I'd suggest the following:
Draw a "spiral curve" on the bolt and a sketch with "a point" on the nut and link the spiral to the point with a simple coincidence relationship...
